Job description

Seeking candidates with healthcare experience and certifications who must be able to work various hours/shifts, that may include weekends and traveling to all 5 boroughs. Tasks include but not limited to:

  • Supporting individuals with disabilities to enhance inclusion to become valued members of their communities
  • Providing direct training and support in individuals’ home and/or community
  • Empowering the individuals and their family by providing the support and information necessary to make informed choices and decisions
  • Help to build self–esteem and assertiveness necessary to lead a self-determining life
  • Use formal and informal methods to learn about the needs, interests and desires of individual
  • Assisting individual in identifying the formal and informal supports available in his/her community, family and social network and works with the individual to develop and sustain relationships with friends, family and community members
  • Assisting the individual in managing every day aspects of life by utilizing habilitative strategies and providing supports in a manner that builds on individual strengths and capabilities and maximizes choices
  • Pursuing opportunities for professional growth and shares knowledge and resources with others *Assisting the individual to overcome challenges through effective advocacy
  • Using appropriate and safe strategies for crisis prevention, intervention and follow up
  • Completing necessary documentation in an efficient and timely manner is required and ensures that program remains in compliance with OMRDD regulations, agency philosophies, and policies
  • Facilitating effective communication with employees and outside agencies for ongoing development of community opportunities
  • Participating in any special case conference, staff and parent meetings as appropriate

  • Organizational skills and good communication skills required
Prior experience & qualifications
  • Must have Healthcare industry experience
  • High School Diploma or Equivalent
