Locations: Various NYC Boroughs (citywide travel may be required, metro cards provided for work related travel) Hours: 9:00am to 5:00pm

Wage: ~$21.53/hr

This position will mostly be field work and home visits with 1 day per week in the office for administrative work. Customer Service representatives will work on a multi-disciplinary team in the Care Network to ensure information received from referral partners is entered into the unit and home visiting systems accurately.

Duties will include:

  • Conducting telephone screenings with clients
  • Providing health education on maternal and infant health issues, including breastfeeding, safe sleep and early childhood development
  • Collecting and entering data; monitoring, reviewing, and managing case assignments and conducting follow-up referrals within 24 hours of assignment
  • Engaging with provider and community-based referral partners, clients and home-visiting program staff to ensure clients are connected to services in a timely manner
  • Supporting marketing and outreach activities to increase visibility and utilization of the unit and home-visiting programs
  • Participating in Program-sponsored events, for example: crib distribution
  • Assisting senior management with other assignments
  • Accompanying home-visitor staff on visits as necessary

What you need to have

  • Data collection experience and familiarity with Microsoft Office applications is needed
  • Excellent written and verbal communication skills required
  • Must be able to travel to different locations
  • Able to walk up steps and carry items that total 10 pounds
Prior experience & qualifications
  • Experience in community outreach, early-childhood development and breastfeeding preferred
  • Experience working with low-income families and/or families in a shelter setting providing maternal-child health education
  • Fluency in English is required. Bi-lingual proficiency in Spanish, Haitian-Creole, French, Mandarin or Russian is preferred
  • High school diploma or GED required
