Job description

  • Help families retain and return to stable housing in the community through crisis counseling medication referral services, benefits advocacy, and collaborative service planning
  • Engage applicants and their primary tenants that present at PATH Family Intake with possible housing option with the goal of stabilizing families in the community
  • Ensure HomeBase referrals are pursued by each family that remains in the community
  • Provide mediation services or feasibility of alternate housing options
  • Provide any rerecommended referrals for ancillary services, as needed
  • Accompany field teams conducting eligibility investigation to assess health and safety condition in primary tenant’s homes when necessary
  • Encourage all families to begin to immediately identify strengths, obstacles, goals and action steps necessary to achieve housing permanence, with the end goal of helping families return to stable housing in the community as quickly as possible
  • Provide feedback/corrective action within prescribed deadlines
  • Upon request, the selected candidates will assist with the preparation of weekly staff time and leave for timekeeping and payroll purpose

  • Excellent interpersonal skills required
  • Computer skills
Prior experience & qualifications
  • Bachelor’s degree-Liberal Arts-Social Work required
  • 1 year of human services experience required
Languages
This opportunity requires fluency in English
